- Utility Bill (Gas, Electric, Water, Land-Line Phone)
- Lease/Rental Agreement (with Landlord’s name, address, phone)
- Current Rent Receipt
- Mortgage Payment, Closing Papers or Mortgage Payment Book
- Residence Insurance Statement
- Notarized Out of School Zone Paperwork
o Must be approved by Nassau County Office prior to enrollment.
LEGAL DOCUMENTS (court orders, custody documents, etc.), if applicable
STUDENT REGISTRATION FORM (Provided by School)
COPY OF PARENT/GUARDIAN DRIVER’S LICENSE
STUDENT DOCUMENTS:
- Birth Certificate
- Immunization Record
- Florida Physical (HRS Form 680 for out of state students
- Most Recent Report Card
- FSA Results (or other Standardized Test Results, if applicable)
- Special Education Paperwork, if applicable
