    • Utility Bill (Gas, Electric, Water, Land-Line Phone)
    • Lease/Rental Agreement (with Landlord’s name, address, phone)
    • Current Rent Receipt
    • Mortgage Payment, Closing Papers or Mortgage Payment Book
    • Residence Insurance Statement
    • Notarized Out of School Zone Paperwork

    o Must be approved by Nassau County Office prior to enrollment.

    LEGAL DOCUMENTS (court orders, custody documents, etc.), if applicable
    STUDENT REGISTRATION FORM (Provided by School)
    COPY OF PARENT/GUARDIAN DRIVER’S LICENSE

    STUDENT DOCUMENTS:

    • Florida Physical (HRS Form 680 for out of state students
