• Registration

     

    We look forward to meeting our future Little Warriors! Currently, registration is by appointment only. The following documents MUST be brought at the time of registration, or your appointment will have to be rescheduled:

     

    • original birth certificate
    • social security card
    • up-to-date immunization record
    • current school physical (must not expire prior to school starting)
    • proof of residency (MUST service the home address: mortgage statement/lease, utility bill, water bill, etc.)
    • parent/guardian identification (MUST have current address)

     

    The packet is available online here, and takes approximately 30 minutes to complete. Please print the packet one sided. If you have not filled out the packet prior to your appointment or need a paper copy to complete, please arrive early to ensure you have enough time.
