NASSAU COUNTY SCHOOLS
Registration for students in kindergarten for the 2022-2023 school year will be held at Bryceville Elementary, Callahan Elementary, Hilliard Elementary, Southside Elementary, Wildlight Elementary and Yulee Primary Schools April 20 – April 26, 2022. Parents may use their private physician or Health Department Clinic to meet immunization and health examination requirements.
AGE REQUIREMENTS: (Florida School Law 1003.21 State Board Rule 6A-1.982, 6A-1.985)
Compulsory Attendance: All children who have attained the age of six (6) years or who will have attained the age of six (6) years by February 1st of any school year are required to attend school regularly during the entire school term.
Kindergarten: A child five (5) years old on or before September 1, 2022, may be admitted any time during the school year.
First Grade: A child six (6) years old on or before September 1, 2022, shall be admitted any time during the school year if the child has satisfactorily completed kindergarten. Transfer students must meet minimum promotion standards required in the Nassau County Pupil Progression Plan.
INFORMATION REQUIRED TO COMPLETE REGISTRATION WILL INCLUDE:
- A birth certificate
- A health examination certificate (Physical Exam) performed within one (1) year prior to enrollment in school signed by a Physician, PA, or DO.
- A certificate of immunization form for those communicable diseases for which immunization is required by the Department of Health Florida Statutes..
- Child’s Social Security Card.
- Proof of residency. (Example, Lease agreement, utility bill, etc.)
IMMUNIZATIONS AND HEALTH EXAMINATIONS:
Parents should get their child’s immunizations with their child’s doctor if they have private insurance or Medicaid. Their physicians will put their data into the FL Shots database and give a certified 680 form.
For those without insurance, immunizations can be provided through the Florida Department of Health, Nassau County at no charge for children birth to 18 years of age through a Federal Vaccine for Children Program. Parents must make an appointment and bring their child’s up-to-date shot record to help identify needed shots and prevent unnecessary shots for their child.
For out of state residents, the Department of Health, Nassau County will provide parents with a required Proof of Immunization Form 680 if the resident does not have a primary care physician in Nassau County. We must have an up-to-date shot record to provide a 680 form.
